Check request form

Download this form to request a check from UUCB for reimbursement of expenses or any other purpose. Send completed forms to the UUCB Office Manager.


Calendar Request Form (“Blue Sheet”)

To reserve a room at UUCB for either one-time or repeated use, download the Calendar Request Form below, fill in required information, print and put in the Executive Director’s box in the office (or save and attach to an email to Tess.)

Note that the form should be submitted or signed by a committee or council chair, Board or CT member, or Minister. Check back if you don’t receive a confirmation soon. Details about room setups can be submitted later, if necessary (at least two weeks ahead of any special event), on the Room Setup Form/Green Sheet. Both forms are also available in hard copy in the front office.


Room Setup Form (“Green Sheet”)

Submit the Room Setup Form below if you will need any chairs, tables, A/V equipment, coffee service, etc. set up by Facilities staff for your calendared event or series of meetings. Submit to Tess as above.


Childcare reservation form for event leaders

To request childcare please download and fill out the form Request UUCB Childcare Services – for event leaders and send it to You may either print out the form and send it by mail, or send it by email attachment.


Childcare reservation form for parents

No reservations are required for Sunday morning childcare or Parents Night Out.

Coming soon: for all other events, parents and guardians will be able to reserve childcare via an online form. Until that is ready, please email your request to
